In /var/log/exim4/paniclog for each received message:

2015-02-16 16:43:43 1YNOlq-0007Jp-Ed malware acl condition: clamd: ClamAV returned: /var/spool/exim4/scan/1YNOlq-0007Jp-Ed/1YNOlq-0007Jp-Ed.eml: lstat() failed: Permission denied. ERROR

The clamav user is in the Debian-exim group, and the clamav process is running as the clamav user, so I'm unsure what's going on offhand.

What are the permissions on @/var/spool/exim4/scan/@ ?

All correct. The issue, it turns out, is this: http://serverfault.com/questions/245229/exim4-wont-send-message

We need to set "AllowSupplementaryGroups yes" in clamd.conf and freshclam.conf

Massive tidy of postinst now sets supplementary groups for clamav-daemon, but ignores freshclam, as I think this is just for database updates, and not relevant to Exim.
